Continuing Education Attendance Procedures • You must arrive on time. • Sign in on the appropriate sign in sheet for your continuing education requirement. • You must furnish the course provider representative with your full name, resident address, and insurance license number with expiration date. • You must attend the course in its entirety. No partial credit will be given. • You must return to the seminar promptly after each break. You are permitted to freely use the facilities but must return to the seminar as soon as possible. • You are not permitted to leave the seminar to make phone calls, smoke or handle other matters. • Newspapers and/or magazines are not permitted. • Telephones, pagers, laptop computers, etc must be turned off. • At the conclusion of each seminar you must personally fill out the course completion form in its entirety and hand it in. These forms are only accepted at the conclusion of the seminar and are not to be left on the tables, faxed, or mailed. No credit will be given unless the course completion form is personally handed in.
